Corticosteroid-Binding Globulin (CBG) is the liver produced protein responsible for binding cortisol, the key hormone involved in processes such as stress response, metabolism, and immune regulation. While total cortisol measurement is widely used in both clinical and research settings, it does not always provide the complete picture. Only free cortisol is biologically active, while the majority is bound to CBG (approximately 80–90%) or to albumin (approximately 5–10%). Measuring CBG is therefore essential for accurate cortisol interpretation, particularly in conditions where CBG levels are altered, such as pregnancy, stress, estrogen therapy, or liver disease.
